Adherence of nurses to annual seasonal influenza vaccination over a 5-year period

背景

医療従事者は、院内伝播と関連するインフルエンザ感染症のリスクがある。流行を制御するために、毎年の季節性インフルエンザワクチン接種に対する継続的遵守が重要である。

目的

季節性インフルエンザワクチン接種に対する看護師の 5 年間の遵守状況とその関連因子を評価すること。

方法

香港における 2019 年 3 月の冬季インフルエンザシーズン後に、看護師を対象とする横断的研究を実施した。2014/15 シーズンから 2018/19 シーズンのインフルエンザワクチン接種率に基づき、回答者を 3 つのグループ、すなわち「完全遵守」（5 シーズンのワクチン接種）、「部分遵守」（1 シーズンから 4 シーズンのワクチン接種）、「非遵守」（ワクチン非接種）に層別化した。毎年のインフルエンザワクチン接種の遵守、回答者の特性、ワクチン接種に対する考えの関連を検討するために、ステップワイズ法による多変量ロジスティック回帰分析を実施した。

結果

募集した看護師 1,306 名のうち大部分は女性（88％）で、年齢中央値は 36 歳（四分位範囲 30 歳から 46 歳）であった。インフルエンザワクチン接種率は、2014/15 シーズンの 36％から 2018/19 シーズンには 47％に増加した。層別化後、回答者のうち非遵守は 39％、部分遵守は 40％、完全遵守は 21％であった。完全遵守は、女性（補正オッズ比［aOR 0.60］）、40歳以上（aOR 2.92）、長期ケア施設の看護師（aOR 0.56）、奨学金支給期間の接種（aOR 3.83）、季節性インフルエンザの地域的流行（aOR 0.51）、専門家の意見（aOR 4.04）と有意に関連した。

結論

季節性インフルエンザワクチン接種を完全に遵守した看護師の割合は限られていた。遵守のモニタリング、ワクチン利用の改善、および遵守率が低い医療従事者を対象とする介入が重要である。

香港からの報告である。医療従事者の季節性インフルエンザワクチン接種の遵守率は国によっては必ずしも高くない。意外かもしれないが、米国は日本ほど高い医療従事者接種率ではない病院が多い。近年日本はこの点が優等生と評価できるが、様々な理由で接種を受けない医療従事者の対応に少なからず苦慮する。こうした遵守に関連した背景を調べ改善をはかることは重要である。
