Incidence and impact of hospital-acquired pneumonia: a Portuguese nationwide four-year study

本稿では、ポルトガルにおける 4 年間（2014 年から 2017 年）の院内肺炎（HAP）の発生率を報告する。ポルトガルの病院 100 施設の成人患者の退院時診断データベースでデータを検索し、性別、年齢、慢性併存疾患、死亡率、入院期間などを抽出した。HAP エピソードは 28,632 例、発生率は入院 100 件あたり 0.95 であった。HAP 患者では、入院の長期化（平均 26.4 日）と高死亡率（33.6％）の両方がみられた。大部分のエピソードが、65 歳以上および男性患者で発生した（それぞれ76.1％、61.7％）。侵襲的人工呼吸器が 18.8％で必要とされた。

監訳者コメント：

ポルトガルにおける院内肺炎（HAP）に関する調査報告であるが、HAPは院内感染における重要な感染症であり、非人工呼吸器装着患者は装着患者の2倍の発生率があるとも言われ、死亡率やICU入室率も同様に高いことが示されている。本論文ではHAPが急性脳血管障害患者において発生率が高く、HAP患者の14．3％を占め、男女差は認められていない。同時にHAPを併発したの血管障害患者の死亡率は4.8倍と高くなっており、脳血管障害による嚥下機能低下が誤嚥性肺炎を引き起こしていると推測されている。さらにHAPの発生率は年齢とともに増加していること、HAPの2/3はICU外で発生していることも重要であり、本論文が初めてポルトガルで実施された4年間にわたる全国的なサーベイランスであり、今後のポルトガルにおけるHAPの基本データとなるであろう。
