Relative faecal abundance of extended-spectrum β-lactamase-producing Enterobacterales and its impact on infections among intensive care unit patients: a pilot study

本前向きパイロット研究では、基質特異性拡張型β-ラクタマーゼ（ESBL）産生腸内細菌目細菌（ESBL-PE）の便中細菌相対量と ESBL-PE 関連感染症の発生との間の関連について評価した。患者 24 例を組み入れた。ESBL-PE 相対細菌量の中央値は32.4％であった。ESBL-PE 相対細菌量の平均は、過去 3 か月間に抗菌薬に曝露された患者では 5 倍以上多かった（P ＝0.002）。さらに、ESBL-PE 相対細菌量の平均は、非・大腸菌（Escherichia coli）株の保菌患者では 2 倍以上多かった（P ＝0.044）。ESBL-PE 相対細菌量の平均は、一致患者では不一致患者よりも 10 倍以上多かった（59.1％対 4.9％、P ＜ 0.001）。

ESBLの 便中相対量を調べることにより、ESBL 産生菌を治療対象とするかどうかの判断に使える可能性があり、さらなる研究が待たれる。
