クロスオーバー試験デザインによる様々な皮膚保護材を用いたハーフマスクレスピレーターの定量的フィットテストの結果の比較：パイロット研究

背景
世界中の臨床医は、新型コロナウイルス感染症（COVID-19）患者を治療するための長時間勤務中にマスクの長期間の使用による皮膚損傷を経験している。皮膚損傷はマスクと皮膚バリアの間での摩擦および圧力の増加の結果である。COVID-19 パンデミック中、臨床医は皮膚損傷を予防し改善するために様々な皮膚保護材を使用してきた。しかし我々の知る限りでは、これらの皮膚保護材をマスクと顔面の十分な密着性を損なわずに使用し安全性と有効性を評価する研究はない。
目的
定量的フィットテストを用いて、様々な皮膚保護材と密着の完全性に関するこれまでで最大規模の研究を実施すること。
方法
本パイロット研究では定量的フィットテストを用いて、シリコン傷跡シート（ScarAway®）、Cavilon™ または Tegaderm™ の装着と保護材なしとを比較して 3M™ ハーフフェイスレスピレータのバリアの完全性に影響するかどうかを調査した。データはニュージャージー州のアカデミックレベル 1 の外傷センター 1 施設の臨床医 9 名から収集した。
結果
シリコン傷跡シートは密着性が最も不十分という結果になった一方で、Cavilon は他の介入と比較して最も高い密着性の要因となった（P ＜ 0.05）。
結論
これらの知見は、COVID-19 パンデミックおよび将来のパンデミック時にマスクを着用する際に、快適性を求めて保護材を検討する臨床医に情報提供するのに役立つ。
監訳者コメント：
コロナ禍において診療や看護時の長時間のマスク装着によって皮膚のダメージが問題になっている。本研究は 3M の half-mask respirator という製品を使用した際に、いくつかの皮膚保護剤の有効性を検討したもので、Cavilon や Tegaderm が最もマスクの密着性を損ねなかったと報告している。若干特定の商品に誘導するような内容であるが、特に COI はないようである。
