尿路病原菌に対する弱有機酸の相乗活性

背景
尿路感染症(UTI)は、もっとも頻度の高い院内感染症の 1 つであり、UTI の 75%は尿道カテーテルに起因すると推定されている。かなりの医療費と患者感染率に加えて、一般的な尿路病原菌において報告される抗菌薬耐性の増大傾向は、喫緊の課題である効果的な新しい抗菌対策の研究を促している。
目的
院内の一般的な尿路病原菌(プロテウス・ミラビリス[Proteus mirabilis]、黄色ブドウ球菌[Staphylococcus aureus]、大腸菌[Escherichia coli]、緑膿菌[Pseudomonas aeruginosa])に対して、一連の弱有機酸(クエン酸、リンゴ酸、プロピオン酸、マンデル酸、乳酸、安息香酸、ピルビン酸、馬尿酸)の単独使用および併用による抗菌活性を検討すること。
方法
最小発育阻止濃度(MIC)、最小殺菌濃度(MBC)、バイオフィルムの最小根絶濃度(MBEC)、fractional inhibitory concentration(FIC)インデックス、弱有機酸の殺菌活性動態を、微量希釈法および時間‐殺菌法により測定した。
結果
試験したすべての弱有機酸は、単独使用の場合、浮遊状態およびバイオフィルム状態で増殖した尿路病原菌に対して殺菌活性を示した。さらに、併用での弱有機酸は、黄色ブドウ球菌、S. aureus、大腸菌に対して相乗活性を示し、MIC 値が 250 倍まで低下し、バイオフィルム形成が有意に低減した。
結論
本研究で確認した多機構の相乗的併用は、カテーテル関連 UTI の治療と予防に重要な役割を果たすことが予測される。

本論文は、尿路感染症の原因となる病原菌に対する有機酸の効果を調べたものである。クエン酸は、一般家庭において除菌やぬめりを除去する効果があるとされているが、どのように臨床に応用していくのかが気になるところである。
