Weak organic acid synergy towards the prevention of catheter blockages

背景

尿道カテーテルを長期留置された全患者の 50％は、カテーテルによる尿路感染症やカテーテル閉塞といったエピソードの再発を経験しており、尿道カテーテルが留置されたままになっている場合、尿閉、腎盂腎炎、敗血症につながる。筆者らは以前に、院内の尿路病原菌に対する弱有機酸併用の相乗活性について報告した。

目的

選択した弱有機酸（クエン酸、プロピオン酸）の単独使用または併用が、結晶性バイオフィルム形成とカテーテル閉塞の予防に及ぼす効果を検討すること。

方法

細菌生存率、尿の pH、カテーテル閉塞までの時間を測定するために、静的結晶化試験および膀胱モデルでの動的 in vitro 試験を走査型電子顕微鏡を用いて実施した。

結果

クエン酸／プロピオン酸を併用した場合、カテーテル先端の小孔周囲の付着物の割合は低減し、閉塞までの時間が 3 倍延長した。

結論

カテーテル関連尿路感染症や関連する閉塞による全体的な医療負担に対処するために、本研究で確認した弱有機酸の相乗的併用は抗菌薬の有望な代替法となる。
