集中治療室における中心ライン関連血流感染症の低減を目的とした毎日のクロルヘキシジン浴の実施

背景
毎日のクロルヘキシジン浴は中心ライン関連血流感染症(CLABSI)の低減と関連している。既に確立している CLABSI サーベイランスシステムおよび CLABSI 予防バンドル実施の状況下で、ICU 患者の毎日のクロルヘキシジン浴が CLABSI 発生率とその原因菌に及ぼす影響を分析した。
方法
本稿はスイスの三次医療施設の集中治療室(ICU)における前後比較研究である。CLABSI とその原因菌の前向きサーベイランスを構築した。介入は中心静脈カテーテルを留置した ICU 患者の毎日のクロルヘキシジン浴とした。19 か月のベースライン期間に続き、9 か月の介入期間を設けた。
結果
患者計 5,008 例を対象とした。ベースライン期間の CLABSI の平均発生率は、1,000 カテーテル日あたり 2.45(95%信頼区間[CI]1.93 ~ 3.07)であり、その後の介入期間では 1,000 カテーテル日あたり 1.00(95%CI 0.55 ~ 1.67、P < 0.001)であった。クロルヘキシジン浴の導入は、CLABSI のリスク低減と独立して関連した(補正オッズ比 0.47、95%CI 0.26 ~ 0.84、P =0.011)。介入期間のセラチア・マルセセンス(Serratia marcescens)の増加を除き、原因菌の有意な変化は観察されなかった。
結論
ICU における毎日のクロルヘキシジン浴の導入は、CLABSI 発生率の低減をもたらした。高所得環境において標準と考えられるベースラインでのCLABSI 発生率と、実施された複数の CLABSI 予防策を起点としたクロルヘキシジン浴は、さらなる低減を図るために有用であることが証明された。
監訳者コメント:
毎日のクロルヘキシジン浴が中心ライン関連血流感染症の低減と関連することはこれまでにも多くの研究で示されているところであるが、実施期間中に観察されたセラチア・マルセセンスの増加は、クロルヘキシジン体制を含め、その理由が大変気になるところである。
