手技関連のエアロゾルの換気テントによる封じ込め:噴霧された薬物粒子の拡散に対する効果

背景
気道に関わるいくつかの医療手技は「エアロゾル発生手技」とみなされている。これらの手技によるエアロゾルは近くにいる人によって吸入される可能性があり、結果として感染症伝播またはネブライザー療法に特有の 2 次的な薬物曝露に関する懸念がある。
目的
商標登録された HEPA フィルタ付き換気テントによる、噴霧された気管支拡張薬のエアロゾル拡散の低減に関する有効性を評価すること。
方法
本研究はアイルランド・ダブリンにある St. James’s Hospital の使用されていない外来患者室 1 室で実施した。新たなリアルタイム蛍光粒子計数器である Wideband Integrated Bioaerosol Sensor によって室内空気を連続的に 3 時間モニタリングした。ベースラインの空気中の粒子数および気管支拡張薬の液剤の噴霧中の粒子数を記録した。
結果
テント内での噴霧はベースラインの値からの増加を防いだ。室内空気への直接の噴霧によって、蛍光粒子数の平均値はそれぞれ4.75 × 105/m3(Ventolin)、4.21 × 105/ m3(Ipramol)となり、ベースラインの平均値と比較して 400 倍超の増加を示す結果となった。薬物粒子の 99.3%超は直径 2μm 未満であり、ゆえに下気道に入れる小ささであった。
結論
換気テントはネブライザー療法由来の吸入できるサイズの薬物粒子の空中拡散を予防するのに十分に効果的であった。このことは、この種の換気テントが新型コロナウイルス感染症(COVID-19)パンデミック中のエアロゾル発生手技による空気感染を予防するのに有効である可能性を示唆している。
監訳者コメント:
PeaceMedical 社のいわゆる「陰圧テント」の有効性を測定可能な薬剤の噴霧によって示した研究。検討自体は 2019 年 7 月に行われており、コロナを想定して行われたものではなかったようだが、コロナ禍によって日の目を見た研究なのかもしれない。
