カルバペネマーゼ産生腸内細菌目細菌を保菌するグラム陰性菌血流感染症の患者における迅速診断ツールの統合★

我々は、カルバペネマーゼ産生腸内細菌目細菌（CPE）の保菌者におけるグラム陰性菌血流感染症（BSI）に対する迅速診断アプローチを導入した。CPE 直腸保菌患者の大規模コホートにおいて、18.1％がグラム陰性菌による BSI を発症し、このうち69.5％が CPE によるものであった。直接マトリックス支援レーザー脱離イオン化飛行時間型質量分析（MALDI-TOF MS）により、腸内細菌目細菌および非発酵グラム陰性菌の同定について、単微生物血液培養陽性においてそれぞれ 97％および53.8％の信頼性をもって可能であった。全体で、多微生物血液培養においても、参照基準法と比較して、NG-Test Carba 5 アッセイの感度および特異度は不一致分析後に100％であった。直接 MALDI-TOF MS と NG-Test Carba 5 アッセイの併用は、CPE 保菌者などの高リスク患者のコホートにおいて、CPE による BSI の迅速診断検査のための信頼性のある費用効果的なツールになると考えられる。
監訳者コメント：
グラム陰性菌血流感染は迅速な診断と治療が重要である。特に CPE は使用できる抗菌薬が限られており、迅速診断の必要性が高い。日本でも MALDI-TOF MS を導入する施設が増えており、本研究は参考になる。
