医療および公衆衛生上のアウトブレイクの調査に全ゲノムシークエンシングを取り入れることの臨床的展望―行き過ぎか、役立つか？★

アウトブレイクは、患者の安全面に大きなリスクをもたらすとともに、調査に費用と時間がかかる。耐性を正確に検出する迅速な結果、および分離株の近縁性に関する臨床的に有用な情報をもたらすタイピングの結果を入手できる感染予防・制御チームによって、的を絞った感染予防・制御策の実施は左右される。現時点で伝播が発生しているかどうかの決定が大きな課題であろう。従来のタイピングの結果は、菌株を明確に示す十分な精度または頑健性が必ずしもあるとはいえず、患者とその環境の関連を確立する十分な疫学的データが入手できるとも限らない。全ゲノムシークエンシング（WGS）は、究極の遺伝子タイピングツールとして誕生したが、調査法とルーチンの微生物学的臨床診断技術との境界線をまだ完全に超えていない。Scottish Healthcare Associated Infection Prevention Institute の活動の一環として、スコットランドから病院環境のアウトブレイクを確認または否定するために、2014 年に WGS 臨床サービスが公式に確立された。本稿では、医療および公衆衛生上のアウトブレイクを調査するために、WGS 使用の実用化に関する新たな洞察を示すことを目的として著者らの経験を報告する。臨床環境でのこの技術の実施を阻む障壁を克服するための解決策を提示する。
監訳者コメント：
全ゲノムシークエンシングによる病原体遺伝子情報の取得は、集めただけでは価値がない。集めた情報を如何に価値ある情報に分析するのかが課題である。コストや時間は日々改善しているが今一段の技術革新が必要だろう。
