背景
基質特異性拡張型β– ラクタマーゼ(ESBL)産生腸内細菌目細菌は、院内感染による世界的な負担をきたしており、いくつかのガイドラインにより、リスク患者のスクリーニングによる保菌者の同定が推奨されている。

目的
迅速 ESBL 検査がスクリーニングの所要時間に及ぼす影響を評価すること。

方法
大腸菌(Escherichia coli)以外の ESBL 産生腸内細菌目細菌を同定するために、直腸スワブを培養と相乗作用試験により分析した。発色酵素基質培地で増殖させたコロニーで迅速 ESBL NP 試験および NG CTX-M MULTI 試験を実施した。ゴールドスタンダードとして PCR および ESBL の遺伝子シークエンシングの結果を用いた。

結果
分析した 473 のスワブ検体のうち 75 検体(15.9%)で、大腸菌以外の ESBL 産生腸内細菌目細菌が増殖し、分離株 89 株を得た。相乗作用試験、迅速 ESBL NP 試験、NG CTX-M MULTI 試験の感度は、それぞれ 0.97(95%信頼区間[CI]0.88 ~ 0.99)、0.81(95%CI 0.69 ~ 0.89) 、0.90(95%CI 0.80 ~ 0.96)、特異度は、それぞれ 0.92(95%CI 0.73 ~ 0.99)、0.85(95%CI 0.64 ~ 0.95)、0.96(95%CI 0.78 ~ 1.00)であった。473 の直腸スワブ検体を相乗作用試験、迅速 ESBL NP 試験、NG CTX-M MULTI 試験を用いて検討することにより、ESBL スクリーニングの性能を算出した。それぞれの試験の感度は 0.96(95%CI 0.86 ~ 0.99)、0.81(95%CI 0.68 ~ 0.90)、0.91(95%CI 0.79 ~ 0.97)、特異度は 1.00(95%CI 0.98 ~ 1.00)、0.99(95%CI 0.98 ~ 1.00)、1.00(95%CI 0.99 ~ 1.00)、陽性適中率は 0.96(95%CI 0.86 ~ 0.99)、0.94(95%CI 0.81 ~ 0.98)、1.00(95%CI 0.91 ~ 1.00)、陰性適中率は 1.00(95%CI 0.98 ~ 1.00)、0.98(95%CI 0.96 ~ 0.99)、0.99(95%CI 0.97 ~ 1.00)であった。大腸菌以外の ESBL 産生腸内細菌目細菌が認められなかった場合、平均所要時間は 30 時間であり、これが同定された場合、相乗作用試験、迅速 ESBL NP 試験、NG CTX-M MULTI 試験の平均所要時間は、それぞれ 74.7 時間、38.0 時間、36.7 時間であった。

結論
ESBL 産生腸内細菌目細菌を保有する患者を同定するために、2 つの迅速 ESBL 試験は良好な性能を示し、スクリーニングプロトコルの所要時間の短縮がみられた。

監訳者コメント
β– ラクタマーゼのうち気質特異性が拡張しているタイプを保菌している耐性菌の場合、治療薬の選択肢が異なる。このため検査室で迅速簡易同定ができれば、抗菌薬療法の適正化が図りやすくなる。
