Borderline oxacillin-resistant Staphylococcus aureus carriage among healthcare workers at neonatal
intensive care unit and paediatric ward

背景
メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus）の接触者追跡調査およびスクリーニング調査中に、新生児集中治療室（NICU）の医療従事者においてオキサシリン耐性が境界値を示す黄色ブドウ球菌（borderline oxacillin-resistant Staphylococcus aureus；BORSA）陽性のスクリーニング培養が 2 件発見された。本知見によりさらなる調査に至った。

目的
医療従事者間の直接伝播によるアウトブレイクの可能性を評価すること。

方法
発見後、感染制御チームが発足した。本チームは追加の感染制御策を開始し、新たな知見を評価した。すべての NICU および小児病棟の医療従事者は BORSA の保菌に関してスクリーニングを受け、患者は前向きに 7 週の BORSA のモニタリングを受けた。医療従事者間の直接伝播によるアウトブレイクの可能性を評価するため、増幅断片長多型および全ゲノムシークエンシング（WGS）を用いて BORSA 分離株を解析した。

結果
陽性の医療従事者はスクリーニングプログラムの結果を待つ間、臨床業務を禁止された。全体で NICU の 127 名および一般小児病棟の 77 名の医療従事者が BORSA の保菌に関してスクリーニングを受け、5 名の医療従事者が BORSA 陽性であった。72 例の患者が 7 週の間にスクリーニングを受け、患者 1 例あたり 1 件から 9 件までの計 138 件の培養が得られた。医療従事者から患者への拡散は起こらず、BORSA のスクリーニングプログラムは中止された。BORSA 株全 5 株の WGS 解析（コアゲノム multi-locus sequence typing）により NICU の 2 株の間の関連性が示された。

結論
7 週の間にスクリーニングにおいても臨床培養においても、BORSA 陽性の医療従事者から新生児への伝播は認められなかった。この脆弱な集団に対して将来エビデンスに基づいた適切な介入をデザインするために、さらなる警戒と経験が必要である。

監訳者コメント：
MRSA は通常 mecA 遺伝子がその原因遺伝子であるが、BORSA はβラクタマーゼの産生と PBP 遺伝子の変異の重複が原因となるらしい。日本ではまだほとんど知られていないが、知識としては知っておくと良いだろう。
