背景
クロストリジオイデス・ディフィシル(Clostridioides difficile)感染症の臨床的負担の重さは、感染患者の迅速で高感度な同定が期待される。しかし、有効な診断は難しいままである。現在の最良の診療ガイドラインは有症状者に対する分子スクリーニングと毒素の直接検出ベースのスクリーニング、またはそのいずれかを推奨しているが、これまでの研究は現存の臨床試験法の一致性と性能に疑問を呈している。
 

目的
C. difficile 臨床分離株のゲノム特性および表現型特性を、C. difficile 感染患者と無症状保菌者の両方における臨床検査結果とより良く関連付けること。
 

方法
単一医療機関の入院患者集団から採取した C. difficile の臨床分離株に関して全ゲノムシークエンシングにより、分子疫学および毒素産生性遺伝子の内容の検討を可能にした。ゲノム所見は臨床検査結果と比較し、複数の診断の相違を特定した。
 

結果
毒素産生性培養は「参照基準」と考えられ、毒素産生性遺伝子の内容の予測において完全な感度と特異度を示した。一方で、Simplexa C. difficile Direct Assay(特異度 100%、感度 88%)、Gene Xpert CD/Epi Assay(特異度 86%、感度 83%)、Quick Check Complete Tox A/B(特異度 100%、感度 30%)に関しては性能の低さが認められた。ゲノム解析は C. difficile 株における毒素遺伝子配列のばらつき、感染患者由来と保菌者由来の分離株の間のゲノム系統学的同等性、環境由来の珍しい C. difficile 系統の保菌者を追加的に明らかにしただけでなく、病原体伝播イベントを追跡する機会ももたらした。
 

結論
これらの結果は、臨床の便検査のアプローチの性能のばらつきだけでなく、全ゲノムシークエンシングが従来の検査アルゴリズムの代替として診断的有用性がある可能性を浮き彫りにしている。

クロストリジオイデス・ディフィシルに対する全ゲノム解析と従来の検査法を比較した論文である。診断に対する有用性はもちろんであるが、アウトブレイクの探知に極めて有用だと思われた。
