Infection prevention and control measures in practices of the Swiss sentinel network during seasonal influenza epidemics

背景
毎年、集団におけるかなりの割合がインフルエンザ様疾患でプライマリケア医を受診するという事実にもかかわらず、プライマリケア診療所におけるインフルエンザ伝播に関するデータは限られている。
目的
スイスのセンチネルネットワークに所属する民間診療所におけるインフルエンザに対する感染予防・制御方法の実施状況を記述すること。
方法
本オンライン横断調査では、2018 年から 2019 年のインフルエンザシーズンに、スイスのセンチネルサーベイランスネットワークに所属する民間診療所 166 施設を対象に、感染予防・制御策に関するデータを収集した。質問は、診療所の状況、感染予防・制御に関する推奨内容、医師およびそのスタッフのインフルエンザワクチン接種状態、手指衛生の遵守、およびマスク着用に関するものとした。
結果
回答した診療所 122 施設（回答率 73.5％）において、回答した医師の 90.2％が自身はワクチン接種を受けているとした一方、すべての診療所で従業員のワクチン接種が勧められているにもかかわらず、医師の 46.7％（120 名中 56 名）は、自身のスタッフのワクチン接種率は推定で 60％超であるとした。大半の診療所（N ＝ 68、55.7％）はスタッフに対して、マスク着用について具体的な推奨を行っていなかった。ほとんどの医師は、患者の診察前（N ＝ 91、74.6％）、診察後（N ＝ 110、90.2％）および医学的処置の前（N ＝ 112、91.8％）に、手指の洗浄または消毒を行っていると報告した。しかしこの割合は、診療所への到着時（N ＝ 78、63.9％）および診療所からの退室時（N ＝ 83、68.0％）にはより低かった。
結論
スイスのセンチネルサーベイランスネットワークに所属する医師のほとんどは、自身はワクチン接種を受けていた。しかし、ワクチンが利用できるにもかかわらず、そのスタッフにおけるワクチン接種率は低かった。手指衛生の指標も十分に高くなかった。これらの結果から、外来診療において感染予防・制御策を実施するためのさらなる取り組みが必要であることが示されている。

監訳者コメント：
新型コロナウイルスが問題になっているが、以前にはインフルエンザが冬の感染症の代表であり、また特に診療所における感染についてはほとんど注目されていなかった。対策は国毎に異なると思われるが、日本でもこういった研究を行っていくべきであろう。
