医療環境における新型コロナウイルス感染症（COVID-19）の伝播は、患者と医療従事者にとって重大な影響を及ぼし、局地的アウトブレイクを増幅させる可能性があり、すでに余裕のないリソースに対してさらなる負担をもたらす可能性がある。COVID-19 の診断における見逃しまたは遅れのリスクは、英国における最初の「封じ込めフェーズ」の間に高かったが、それは検査を行う基準が厳格であったためである。無症状／発症前の患者からの伝播に加えて、臨床検査が直面する課題が伴い、そうしたリスクが依然として残っている。筆者らは、ロンドン南西部の大規模急性期国民保健サービス（NHS）トラストにおける COVID-19 の初発症例に関連して発生した可能性のある院内伝播について症例研究を提示し、広がりつつある院内感染による負担について記述する。

監訳者コメント：
COVID-19 の院内感染対策は日本でも課題であり、無症状/発症前の感染者から伝播するCOVID-19 において、検査を行う基準は緩めに考えていたほうがよいと思われる。
