Consensus demonstrates four indicators needed to standardize burn wound infection reporting across trials in a single-country study (ICon-B study)

緒言
熱傷創感染症の治療のためにエビデンスに基づく介入が必要である。試験間での熱傷創感染症の報告に使用される指標のばらつきによって、エビデンスの統合が制限されている。熱傷創感染症の一貫した報告は、コンセンサスにより最小限の指標を設定することでより促されるであろう。Infection Consensus in Burns study（ICon-B study）の目的は、熱傷創感染症のコアセット指標について専門家のコンセンサスを得ることである。
方法
システマティックレビューおよび専門家の意見により特定した熱傷創感染症の候補指標のリストの作成によって、コアセット指標を確立した。デルファイ法による調査では、英国の専門家の参加者が、熱傷創感染症患者における日常診療での使用・診断における重要性・観察の頻度に照らして指標を評価した。熱傷創感染症患者においてその指標が診断に重要であること、また日常診療で使用されることに参加者の 75％以上が合意した場合と、その指標の観察頻度が高いと参加者の 50％以上が評価した場合、コアセット指標に組み入れた。
結果
システマティックレビューにより 195 の指標が確認され、同じ意味の指標を 1 つにまとめて 29 の調査指標に減らした。デルファイ法による調査には英国の専門家75名が参加した。第一ラウンドと専門家パネルによるコンセンサス会議を経て、コアセット指標に次の 4 つの指標を組み入れた。発熱、紅斑の拡大、白血球数の変化、病原菌の存在である。
考察および結論
エビデンスの統合を容易にするために、一国内で専門家の情報に基づく系統的アプローチを実施し、熱傷創感染症のアウトカムを報告する試験において一貫して報告されるコアセット指標を開発した。今後の研究では、国際的な専門家によるコアセット指標の検証が必要である。
