Economic analysis of healthcare-associated infection prevention and control interventions in medical and surgical units: systematic review using a discounting approach

院内感染症または医療関連感染症（HCAI）は、世界的に見ても、患者と医療施設の両方に影響を及ぼす経済的負担と関連する。手指衛生、衛生状態・衛生設備、スクリーニング、基本的・追加的予防法に関する最善の臨床ケア実践は、経済的負担の低減を図るものである。COVID-19 のパンデミックから、これらの 4 つの最善の臨床ケア実践は、HCAI の拡散を抑える極めて重要な予防策であることが確認されている。本稿では、割引アプローチを用いたこれらの 4 つの最善の臨床ケア実践に関連する経済的評価のシステマティックレビューを実施した。2000 年から 2019 年に発表された論文を検索した。クロストリジオイデス・ディフィシル（Clostridioides difficile）関連下痢症、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus）、バンコマイシン耐性腸球菌（vancomycin-resistant enterococci）、カルバペネム耐性グラム陰性桿菌の感染予防・制御に関する経済的評価を対象とした。費用最小化、費用対効果、費用効用、費用便益、費用結果の分析により結果を検討した。論文の質について評価した。計 11,898 報の論文をスクリーニングし、7 報を対象とした。大半の研究（7 報中 4報）の質は、全体的に中程度の質であった。すべての研究で、最善の臨床ケア実践の費用対効果が確認されていた。最善の臨床ケア実践による年間の正味費用削減の平均は、252,847 ドル（2019 年カナダドル）から 1,691,823 ドルと幅があり、割引率（3％、8％）に依存した。最善の臨床ケア実践の増分便益費用比の平均は 2.48 ～ 7.66 であった。資源の有効利用と健康上の利益の最大化を図るために、感染制御に関する経済的評価にあたり、エビデンスに基づく医療方針決定を支援する継続的な研究を実施すべきである。
