Intranasal octenidine and universal chlorhexidine bathing can reduce meticillin-resistant Staphylococcus aureus acquisition in an extended care facility in Singapore

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）は長期ケア施設で蔓延している。我々は 2013 年 1 月から 2019 年 6 月に 100 床のリハビリテーション病院 1 施設において準実験的前後比較研究を実施した。この期間を通じて患者全例に対するクロルヘキシジン浴が実行され、2017 年 9 月からは MRSA 保菌者に対するオクテニジンの鼻腔内投与が追加された。分割時系列分節回帰分析によって、入院時の MRSA の保菌と手指衛生遵守の調整後、MRSA 獲得に関してオクテニジンの鼻腔内投与の実行前に一定のトレンドが観察され（調整係数の平均 0.012、95％信頼区間［CI］－0.037 ～ 0.06）、実行時に即時の減少が見られ（－2.145、95％CI －0.248 ～ －0.002、P ＝ 0.033）、続いて実行後に有意な減少が見られた（－0.125、95％CI －0.248 ～ －0.002、P ＝ 0.047）ということが明らかになった。

監訳者コメント：
MRSA 保菌は、長期リハを実施するリハビリテーション病院においては重要な問題であり、鼻腔や皮膚の保菌は感染源となるため、除菌することが効果的とされている。鼻腔は MRSA の重要なリザーバーであるため、ムピロシンによる鼻腔除菌とクロルヘキシジン浴の併用が MRSA 除菌には有効であることが認識されているが、ムピロシン耐性が問題化している。オクテニジンによる鼻腔の除菌効果についての研究は不十分であり、本論文ではオクテニジン鼻腔塗布とクロルヘキシジン浴の併用によるMRSA 除菌効果を院内でのMRSA 獲得率で検討し、その効果を確認している。
