背景
2005 年から 2012 年まで大幅に減少したにもかかわらず、院内発症メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)血流感染症(BSI)は病的状態と死亡の主な原因であり続けている。
目的
院内発症 MRSA BSI のリスク因子と根本的な原因を示すこと。
方法
本研究では高負担の短期療養型急性期病院 8 施設において院内発症 MRSA BSI を調査した。症例は、その前の 2 週間に MRSA 陽性の血液培養がない患者より 2016 年に入院 4 日目またはそれ以降に採取された血液検体からの最初の MRSA の分離と定義した。症例の人口統計学的データとリスク因子は診療記録の抽出によってレビューした。可能性のある感染症の臨床的原因は臨床医の委員会の合意によって決定された。
結果
適格症例 195 例のうち 186 例を調査した。症例はほとんどが男性(63%)であり、年齢中央値は 57 歳(範囲 0 ~ 92 歳)であった。BSI の前の 2 週間において症例の 88%は留置器具を有しており、31%は外科手術を、18%は透析を受けていた。最もよく見られた発生場所は集中治療室(ICU、46%)と step-down unit(19%)であった。最も多く特定された互いに排他的ではない臨床的原因は中心静脈カテーテル(46%)、非外科的創傷(17%)、手術部位感染症(16%)、人工呼吸器関連でない医療関連肺炎(13%)、人工呼吸器関連肺炎(11%)であった。
結論
器具および手術関連の感染症は、院内発症 MRSA BSI のよく見られる原因であった。これらの施設での院内発症 MRSA BSI の負担を減少させるために、器具および手術関連の感染症の現在の予防バンドルの遵守改善、ならびに ICU 患者や特定の留置器具を有する患者・特定の高リスクの手術を受ける患者に対する感染源管理に重点を置いた予防戦略が求められている。

監訳者コメント
本論文の考察でも触れられていたが、過去 20 年近くにわたり対策が計画、実行され、耐性菌の問題は軽減されてきたものの、未だに MRSA 血流感染は院内感染の罹患率、死亡率に関与している。論文の最後に、昨年 CDC より発表された MRSA フォーラム BSI に関するガイドラインが紹介されていた。
