産業革命期に、古典的な水系病原体（コレラ／腸チフス）によって、安全な給水と衛生設備の開発が促された。その設備はこれらの病原体には効果的であったが、その他の細菌が建物の狭い水道管にバイオフィルムを形成する可能性を与えた。1976 年にレジオネラ菌が発見された。1967 年まで遡るエビデンス（1995 年、マンチェスターに於て、シンクからの飛沫で経静脈栄養が汚染されたことによる乳児死亡など）にもかかわらず、他の施設内水道配管日和見病原体の水系伝播であると認められるには、英国の医療サービスにおける2011年の新生児 4 例の死亡と報道機関の力を要した。医療施設の建設業界は概して、人間が本来備えている水の安全性への関心を欠いており、伝播に関する認識不足は、水系感染症による感染／死亡回避を進める上で大きな妨げとなる。強い耐性を示すグラム陰性菌の出現は、現代の排水システムのさらなる不備を浮き彫りにしている。これらの細菌はその拡散を高める特別な適応性がないと考えられ、われわれは単に、検出されずにいる感受性微生物がよくたどる経路に注目する。O’Neill レポートは、効果的な抗菌薬のない暗い将来について警告している。本レビューでは、現代の給水／衛生設備が、依然として患者の安全性（および公衆）へのリスクを示している理由に関してエビデンスを検討し、それらの設備が、コレラの現代版、すなわち抗菌薬耐性の拡散をもたらすなら、その設計に不備な点がありうることを示す。

耐性菌の自然環境循環はヒト – 環境 – ヒトのみではなく、家畜等の動物をを介しても起こっている。生態系を崩さないようにしながら環境への抗菌薬による選択圧を減らすことが今後重要となる。
