RESIST-5 O.O.K.N.V. and NG-Test Carba 5 assays for the rapid detection of carbapenemase-producing Enterobacterales from positive blood cultures: a comparative study

本稿では、特徴づけられた腸内細菌科細菌分離株 130 株を添加した血液培養検体から直接、RESIST-5 O.O.K.N.V. アッセイと NG-Test Carba 5 アッセイの性能を前向きに比較した。全体的に見て、両アッセイとも、KPC 型カルバペネマーゼおよび OXA-48-like カルバペネマーゼを検出する感度は 100%であった。KPC-31 および KPC-33(それぞれ、KPC-3 および KPC-2 の D179Y 点変異バリアント)は、両アッセイにより検出できなかった。これらは、カルバペネマーゼ活性は低いが、セフタジジム・アビバクタムに対する耐性を示す。血液培養からの細菌ペレットにおいて、NDM 型および VIM 型のカルバペネマーゼが、RESIST-5 O.O.K.N.V. アッセイにより、それぞれ 50.0%、52.2%検出されたのに対し、NG-Test Carba 5 アッセイでは 100%検出された。4 時間の初期継代培養によって、RESIST-5 O.O.K.N.V. アッセイを実施することにより、このアッセイの感度はそれぞれ 100%、95.6%に改善した。

カルバペネマーゼ産生腸内細菌科細菌(CPE)の検出頻度が高い地域では本研究のような血液培養検体での迅速検出が重要となるだろう。
