Effects of hand disinfection with alcohol hand rub, ozonized water, or soap and water: time for reconsideration?

擦式アルコール製剤の大腸菌（Escherichia coli）に対する除菌効果を試験し、オゾン処理した水道水または石けんと水を用いた手洗いと比較した。アルコール製剤は参加者 35 人中 10 人においてすべての細菌を除菌したが、消毒後の残存の平均は 2,330（標準偏差［SD］4,227）cfu/mL であった。一方、オゾン水は参加者 55 人中 10 人においてすべての細菌を除菌し、残存の平均はわずか 538（SD 801）cfu/mL であった（P ＝ 0.045）。石けんによる手洗いは最も効果的であり、参加者 20 人中 6 人において細菌を完全に除菌し、残存の平均は 98（SD 139）cfu/mL であった（P ＝ 0.048［対オゾン水］、P ＝ 0.018［対アルコール製剤］）。

監訳者コメント：
WHO では医療関連感染の予防にアルコールによる手指消毒を強く推奨している。ただし、肉眼的に汚れがある場合やアルコールに無効な病原体に曝露した後などには石けんと流水による手洗いを行う必要性がある。本研究では有効性においてアルコールによる手指消毒が石けんと流水による手洗いに劣っているが、アルコールによる手指消毒は短時間でほとんどの微生物を除去でき、特別な設備が必要ないことなどから、この結果をもって石けんと流水による手洗いを優先させることにはならない。また、オゾン水による手指消毒は一般的ではないが、石けんと流水による手指消毒に劣っており、改めて導入を検討する必要はないだろう。
