目的
南アフリカ共和国・クワズール・ナタールの 4 つのレベルの公立病院におけるサルモネラ(Salmonella)属菌の拡散を調査すること。
方法
Salmonella 属菌の同定は invA 遺伝子の増幅によって行った。分離菌に対して抗菌薬感受性試験、ならびに 8 つの耐性遺伝子(qnrA、qnrB、qnrS、tetA、tetB、tetC、tetG、ermB)と 3 つの病原性遺伝子(sitC、spvA、spv)の分子特性化を行った。分離菌間の遺伝的関連性は enterobacterial repetitive intergenic consensus(ERIC)-PCR 法を用いて明らかにした。
結果
94 の分離菌を採取した。分離菌の最大の供給源は地域病院であった。小児病棟は分離菌の分布率が最も高かった。看護師の机はサンプル採取を行ったすべての場所の中で最も多くの分離菌を含んでいた。分子タイピングによって、多様な分離菌を示す 22 のクラスターを得た。4 つの主要な ERIC 型が同定され、それぞれが特定の病院に固有であった。複数の病棟内の様々な場所で高度に近縁の分離菌が存在していたので、病棟を超えた拡散の可能性が示された。これらの場所の多くは医療スタッフの出入りが多い場所であった。10 の多剤耐性分離菌が確認された。
結論
本研究によって、環境の清掃と汚染除去を含む感染制御策は Salmonella 属菌の拡散を防ぐのに十分でない、または十分に遵守されていない可能性があることが示唆された。

監訳者コメント:
サルモネラ属菌のような腸内細菌科細菌は、環境汚染の影響は比較的少ないと考えられているが、本研究では、看護師の机、電話、血圧計などかなり広範囲に環境汚染が見られることを示した。日本でも薬剤耐性腸内細菌科細菌の増加が問題になっているが、その対応として、環境汚染への対処にもさらなる配慮が必要かもしれない。
