Impact of isolation precautions on quality of life: a meta-analysis

隔離予防策が患者の精神的健康状態に及ぼす影響は、いまだ十分に定量化されていない。本研究の目的は、隔離予防策が患者の健康関連 QOL および抑うつまたは不安の尺度に及ぼす影響を評価するとともに、不安と抑うつに伴う 1 日あたりの費用を推定することであった。隔離予防策の影響に関する文献を EMBASE および PubMed のデータベース、Google Scholar で検索した。論文の 2 段階スクリーニングを独立して実施した。QOL および精神的負担のさまざまな尺度を用いて隔離患者と非隔離患者を比較した論文を対象とした。Hospital Anxiety and Depression Scale（HADS-A［不安］、HADS-D［抑うつ］）を用いてメタアナリシスを実施した。選択した文献の精神的負担尺度のスコアを効果量としてグラフに示した。不安と抑うつに伴う 1 日あたりの費用をメタアナリシスによるプール平均差を用いて推定した。106 報の論文のうち 94 報は選択基準に満たないため除外され、残る 12 報を全文レビューによるスクリーニングの対象とした。論文の全文レビュー後、実証的分析の結果を報告した 7 報が最終的に残り、この 7 報のうち 4 報がメタアナリシスの対象となった。HADS-A のプール平均差の推定値は －1.4（P ＝ 0.15）、HADS-D のプール平均差の推定値は －1.85（P ＝ 0.09）であった。精神的負担尺度の実証的分析において、1 報を除くすべての研究で負の影響が示された。精神的負担のメタアナリシスと実証的分析の結果より、全体的に隔離患者の方が深刻な状況にあることが示唆された。質調整生存年の観点から、不安と抑うつの 1 日あたりの推定費用は約 10 US ドルであった。

患者だけでなく、医療従事者の精神・肉体的ダメージについても調査して欲しかった。
