背景
人工呼吸器関連肺炎のサーベイランスは、一部の症例は予防できうるが死亡の要因となる可能性もあるという懸念が理由で、質の指標の 1 つとして用いられている。全国的な報告のためにさまざまなサーベイランスの基準が存在しているが、大規模な直接比較は実施されていない。
方法
前向きコホート研究では、欧州疾病対策センターと米国疾病対策センターの、人工呼吸器関連肺炎に対してルーチンに使用される 2 つのサーベイランスの基準を、大規模な総合集中治療室 2 室に入院しているすべての患者に適用した。診断率および診断事象間の一致を比較した。
結果
研究期間中にリスクのある患者計 713 例が特定された。欧州のサーベイランスアルゴリズムは、1,000 人工呼吸器日あたり人工呼吸器関連肺炎 4.6 例(95%信頼区間[CI]3.1 ~ 6.6)の診断率を返し、米国のサーベイランスシステムでは診断率は 1,000 人工呼吸器日あたり 5.4 例(95%CI 3.8 ~ 7.5)であった。診断事象間の一致は不良であった(Cohen のカッパ係数 0.127[-0.003 ~ 0.256])。
結論
アルゴリズムは同程度の診断率を算出したが、事象の一致の不足により人工呼吸器関連肺炎の診断に対するアルゴリズム間の不一致が明らかになり、治療の質の指標としてのサーベイランスの役割を損ねる可能性がある。

監訳者コメント:
人工呼吸器関連肺炎のサーベイランス基準はいまだに議論の絶えないところである。本研究でも陽性率は類似した結果であったが、該当した症例の一致率は低かった。何のためのサーベイランスかを明らかにし、その目的に合致した症例を拾い上げることができるような基準作りが必要である。
