Handwashing sinks as the source of transmission of ST16 carbapenem-resistant Klebsiella pneumoniae, an international high-risk clone, in an intensive care unit

カルバペネマーゼ遺伝子 bla_NDM-5 を保有しているカルバペネム耐性肺炎桿菌（carbapenem-resistant Klebsiella pneumoniae；CRKP）シークエンス型 16 の分離株は、集中治療室 1 室の患者 2 例において院内血流感染症または腸管保菌を引き起こした。手洗いシンクが感染源であるという仮説を立て、集中治療室のすべての手洗いシンクのサンプル採取を行った。全ゲノムシークエンシングと解析によって、1 つのシンクが両方の患者の CRKP の保菌／感染源であり、患者間の共通クローンの直接感染ではないことが明らかになった。本研究は、手洗いシンクが多剤耐性病原体の重要な感染源であることを浮き彫りにした。体液の廃棄の禁止および塩素による日常の消毒を含むシンクの管理によって、感染が抑制された。

近年相次いでいる「シンクを原因とする」CRE のアウトブレイクの報告である。問題となるのはシンクの管理であるが、本論文では体液の廃棄を厳しく禁じたことと、毎日 2,000 ppm の塩素溶液を 5 L 流したとある。
