インドにおける患者家族の手指衛生遵守：非公式の「第 4 カテゴリー」の医療従事者を教育することの重要性

緒言
インドでは人材不足のため、近親者や親しい人に対する医療行為に、患者の家族が積極的に関わることがよくある。患者の家族は、世界保健機関（WHO）の「手指衛生の 5 つのタイミング」すべてにおいて患者とかなり接触している。本研究では、10 年におよぶ啓発キャンペーンが、患者介護者の手指衛生遵守に及ぼした影響を分析した。
方法
2014 年 1 月から 2018 年 12 月にかけて、さまざまな病院環境の患者付添人を対象に、訓練を受けた感染制御看護師が 5 つのタイミングの各時点における手指衛生遵守について観察した。イベント数を総タイミング数で除したパーセンテージとして遵守率を測定した。
結果
計 7,302 回のタイミングが観察され、全遵守率は 46.1％（2014 年の 35.5％から、2018 年には 48.2％、P ＜ 0.0001）であった。WHO によるタイミング 1、2、3、4、5 における遵守率は、それぞれ 51.0％、47.4％、67.6％、48.8％、24.3％であった。家族間では、新生児の母親の手指衛生遵守率（77％）が、その他（44.5％）と比べて極めて高かった（P ＜ 0.0001）。さらに、外科病棟よりも内科病棟で、成人病棟よりも小児病棟で遵守率が高かった（いずれもP ＜ 0.0001）。
結論
本稿は、低中所得国の病院環境における家族の手指衛生遵守に関する最初の研究である。この研究より、家族が教育を受けると、患者の介護をする際の手指衛生の遵守状況がかなり良好となり、とくに新生児の母親において顕著であることが示された。患者の付添人に手指衛生の重要性と方法について教育し、自立を促すことは、患者にとって多大な利益となる可能性があるので有意義である。
監訳者コメント：
Joint Commision Internationalのサーベイの基準と照合すると、観察されたタイミングが十分ではないかもしれない。いずれにせよ、計測することが目的になってはならない。サーベイランスは結果評価であり、部署の特徴にあわせた改善プログラムを実施することが重要である。
