超多剤耐性菌の保菌期間は、回収するのが難しい可能性がある複数の因子に依存している。我々は回収しやすいパラメータ（連続したスクリーニング陰性の回数）を用いて超多剤耐性菌 の保菌期間を評価することを目的とする。131 例の超多剤耐性菌保菌者（バンコマイシン耐性エンテロコッカス・フェシウム［Enterococcus faecium］ 51 例、カルバペネマーゼ産生腸内細菌科細菌 80 例）を対象とした。連続したスクリーニング陰性の回数は超多剤耐性菌の消失に強く関連していた。7 か月間にわたり 3 回以上のスクリーニング陰性を示した患者はすべて、その後にスクリーニング陽性を示すことはなかった。個別的なリスク評価の一部としてスクリーニング陰性の回数を考慮することは、超多剤耐性菌に焦点を合わせた予防策を継続するか解除するかという決定に役立つ可能性がある。

監訳者コメント：
VRE や CRE は長期間にわたって保菌が続くため、「一度検出されたら生涯接触予防策をとる」とする意見もある。しかし本研究では、保菌検査で連続して陰性になることがその後の菌消失と相関したとしている。
日本でもこれらの耐性菌に対してはタイミングをみて保菌検査をすることが有用かもしれないということだろうか。
