手術部位感染症（SSI）の監査にさまざまなレベルで関与した 6 名の外科医との面接により、全国的な「Getting It Right First Time：最初に正しく医療を行う」の実践について評価した。有益な影響は、専門医間の新たな連携の構築、利害関係者関与の改善、SSI プロファイルの増加であった。浮き彫りにされた特定の知識格差は、参加者によってSSI 診断基準の要点までは認識していないということであった。方法論的な不備のため、収集したデータの質は不良と思われた。確保された時間がなく、責任が具体的に示されていない修練医に頼ると、監査は多大な時間を要し、プロセスを維持できないとされた。

監訳者コメント：
“Getting It Right First Time（GIRFT）”のコンセプトを一言で説明するのは難しいが、医療において不必要な回り道を削減し、「最初に」正しく医療を行うための活動である。イギリスでは SSI を含め 40 以上の医療の分野で取り組みが行われている。しかし本論文は、実際に担当者に面接を行うと、現実は理想とずいぶん解離がありそうだ、という内容である。”Choosing wisely”と若干似たところはあるが、”GIRFT”のコンセプトは知っておくと良いかもしれない。
