背景
病院環境における多剤耐性菌伝播の制御のために、高頻度接触面の清掃はきわめて重要な措置である。とくに資源の乏しい施設では、簡便に実施可能で、信頼性のある技術により清掃方法が科学的に監視されれば、その方法はもっとも効果的になるであろう。

目的
病院環境の高頻度接触面の清掃の評価において、既存の市販蛍光マーカーシステムに匹敵する新規蛍光マーカーを特定するとともに、その有効性を評価すること。

方法
洗浄目的に使用される液体洗浄剤を、新規蛍光マーカーとした。さまざまな患者治療区域の高頻度接触面 250 カ所における清掃前後のサンプリングを、新規蛍光マーカーと好気性菌コロニー数を用いて実施した。2 つの方法間の一致率を Cohen のκ係数により評価し、比較した。微生物学的方法に対して、新規蛍光マーカー法の感度、特異度、陽性適中率、陰性適中率を算出した。

結果
2 つの方法間の全体的な一致率は 79.6%で、一致性は良好(κ = 0.60)であることが認められた。蛍光マーカー法の感度は 79.58%(95%信頼区間[CI]72 ~ 85.85%)、特異度は 79.63%(95%CI 70.79 ~ 86.78)、陽性適中率は 83.70%(95%CI 76.38 ~ 89.50)、陰性適中率は 74.78%(95%CI 65.83 ~ 82.38)であった。

結論
資源の乏しい施設での毎日の環境清掃法の評価において、本研究で使用された蛍光マーカーは、市販の蛍光マーカーに対して簡便かつ費用対効果の高い代替品となることが証明された。今回の蛍光マーカーの使用が一般化される前に、このマーカーと既存の蛍光マーカーを直接比較するさらなる研究を推奨する。

監訳者コメント

米国では新規蛍光マーカーを用いたところ、従業員がブラックライトを購入してきて、マーカーのあるところだけを清掃するようになったという事象が報告されている。シミュレーションとしては良いだろうが、リアルな現場には信頼関係も重要である。
