待機的ストーマ閉鎖術（stoma reversal surgery）における陰圧閉鎖療法：英国の地域総合病院におけるパイロット研究の結果★

ストーマ閉鎖手術における陰圧閉鎖療法の役割については、依然として不明である。その役割について評価するために、後向き非無作為化単一施設パイロット研究を実施した。手術創閉鎖術の術式および単一ブランドの陰圧閉鎖療法デバイスの選択は、外科医の選好に任せた。介入群では創合併症をきたした患者はいなかったが、対照群では 36 例中 5 例が手術部位感染症に関連する合併症をきたした。1 次創閉鎖術および陰圧閉鎖療法の使用により、ストーマ閉鎖手術における創合併症が減少し、これにより病院および地域における創管理の負担が減少する。このことには費用削減における意義があるが、さらなる研究が必要である
監訳者コメント：
ストーマ閉鎖手術後の創部感染率は高く、術後の入院期間や医療費の増大、患者への精神的負担などが大きなリスクとなり、再手術や再入院を余儀なくされることがある。英国のNICEは陰圧閉鎖療法（NPWT）を全般的な外科的手術後の創閉鎖に使用するよう推奨しているが、ストーマ閉鎖手術でのNPWTに関するデータは極めて少なく、今後の大規模な前向き研究が必要である。
