Financial and temporal costs of patient isolation in Norwegian hospitals

背景
抗菌薬耐性菌の保菌患者または感染患者の隔離は、ノルウェーで実施される感染制御策の一つとして確立している。この隔離費用に関して、地域の信頼性のあるデータが必要である。

方法
3 つの急性期総合病院の感染症病棟において、医療の観点からのミクロ原価計算法による研究を、直接観察、職員登録、面接、調査データを用いて実施した。

結果
隔離による 1 日追加費用は、非寝たきり患者では 56.8 ユーロ（95％信頼区間［CI］42.4 ～ 72.7）、寝たきり患者では 87.5 ユーロ（95％CI 48.3 ～ 129.6）であった。これら合計費用のうち、労働費が最大の割合を占め（71 ～ 72％）、次いで、個人防護具費（21 ～ 23％）、廃棄物管理費（6 ～ 8％）であった。全体で、隔離特有の業務量は、非寝たきり患者では 1 日あたり 65 分、寝たきり患者では 1 日あたり 95 分で、主に、看護師による延長時間が占められた寝たきり患者のより高額な隔離費用は、主に個人の衛生の実践に使用される資源によるものであった。隔離されていた患者の退院後の病室清掃による 1 回限りの費用は、平均 14.0 ユーロ（95％CI 10.7 ～ 17.6）であった。

結論
本研究は、病院における患者隔離による資源の使用に関して新たな詳しいエビデンスを提示するものであり、衛生上の予防措置に対する今後の評価に情報を提供するために使用が可能である。今回の結果は、隔離患者が多い病棟への看護職員の追加配置を検討すべきであることを示唆している。
