背景
造血細胞移植患者の病室の病原体の抑制により病院感染の予防を図るために、環境衛生はもっとも重要な対策の一つである。本研究は、JACIE による微生物学的モニタリング要件を受けてデザインし、予防隔離室の環境衛生を評価することを目的とした。
方法
好中球減少度が異なる患者の病室において、静止条件および作動条件で対象表面から採取した微生物量、ならびに受動的・能動的空気サンプルの微生物量の算出により環境の清浄度を評価した。さらに、微生物量が隔離予防策により影響されるかどうかも評価した。
結果
清浄度の失敗率は、対象表面については静止条件では 0％であったのに対して、作動条件では 37％であり、受動的・能動的空気サンプルについては 13％であった。好中球減少度が異なる患者の病室において失敗率の差が認められた（表面ではP ＝ 0.036、受動的空気サンプルではP ＝ 0.028）。感染症と微生物量との相関はみられなかった。
結論
病院衛生のモニタリングプログラムの強化と統合した微生物学的評価は、造血細胞移植患者における感染制御策の推進のために極めて重要な情報をもたらす。これらの結果は、臨床施設における清浄度評価のための厳格な基準の設定、および妥当性の確認を行う必要があることを強調するものである。
