「一律の方法を万人に適用するアプローチ」からオーダーメイドの感染予防へ

100 年前には、多くの医療関連感染症（HAI）は外因性のものであり、これは、他の患者、医療従事者、または病院環境由来の微生物によって HAI が引き起こされたことを意味する。患者自身の微生物叢由来の内因性微生物に起因する割合は低かった。その間、多くの改善が成されてきた。患者の診断および治療に使用される器具のほとんどが、今では使い捨てのものである。それ以外の器具の再処理に使用される消毒法および滅菌法は、安全度が非常に高く（少なくとも高所得国では）、器具による交差伝播はごくまれな事象である。手指衛生も実質的に改善している。ドイツでは過去 12 年間に擦式手指消毒薬の消費が増加し、100％を超えている。過去の対策では、患者の疾患や病原体に関係なく、すべての患者にそれらを適用せざるを得ないため、「一律の方法を万人に適用するアプローチ」が妥当となる。
今日では、外因性 HAI の割合は低いとわれわれは理解している。大多数の HAI は内因性のものである。もちろん、微生物の伝播を予防するために、基本的または標準的な感染制御策に主眼を置くことは今なお必要である。だが、現在、大多数の HAI は内因性のものであるため、対策として、微生物伝播の予防だけでなく、感染予防にも重点的に取り組む必要がある。そこで、今こそオーダーメイドの感染予防アプローチが妥当となり、その実例を提示する。
監訳者コメント：
施設内病原体伝播がいつ誰から誰に起こるのかが分からないのが現代医療であることを考慮すると、すべての患者に一律に同じような感染予防策をとることが施設内伝播を阻止する切り札となる。手指衛生に代表させるような感染予防策はまさにこれにフィットする。
