Antibiotic decision making in surgical intensive care: a qualitative analysis

背景
病院、特に外科専門領域および集中治療室において抗菌薬は大量に使用されている。外科医と集中治療医による抗菌薬使用に、抗菌薬適正使用支援プログラム（ASP）は、次第に介入しつつある。しかし、重症外科患者が綿密な観察下に置かれうる病院診療の区域である、外科集中治療室（SICU）における抗菌薬の意思決定を特徴づけるものに関する情報は限られている。
目的
SICU における抗菌薬の意思決定を特徴づけるものを調査すること。
方法
アメリカの教育病院 2 施設で、計 160 時間の参与観察および 10 件の半構造化面接を実施した。データはテーマごとのコード化を用いて分析した。
結果
抗菌薬使用に関して、SICU の診療の 3 つの重要な特徴を明らかにした。（1）物理的な近接は SICU の臨床医に患者の状態の変化を強く認識させる、（2）患者の状態の情報伝達は SICU の臨床医による積極的な関与に依存している、（3）SICU の臨床医は抗菌薬の決定に関して、論争となり変動しうる自己決定権を有している。
結論
SICU における抗菌薬の意思決定は、患者症例に対するさまざまなレベルの物理的な近接と自己決定権を伴う複数の臨床医のチームを含む複雑なプロセスである。本研究により、SICU の臨床医のチームは患者症例に対する物理的な近接が増えているが、抗菌薬の決定に関する自己決定権はほとんどないことが確認された。これらの特徴が考慮されない場合、抗菌薬適正使用支援の介入によって、SICU での大量の抗菌薬使用への対処の成功が損なわれる可能性がある。
