Use of health databases to deal with underreporting of surgical site infections due to suboptimal post-discharge follow-up

本研究では、イタリアの一地域において手術部位感染症（SSI）に関して実施されている特別なサーベイランス（SIChER）とその他のデータソースを併せ、さらに少数の症例を対象としたレビューを統合した、統合サーベイランスについて述べる。術後のフォローアップに関し、退院データ、微生物検査データおよび救急部のデータベースから追加の情報を取得した。これらのデータに基づき、患者 76 例をSSIの可能性のある症例として再分類し、医療トラストの代表者にリバイスをしてもらった。最終的に SSI の新規症例として 45 例が確認され、これにより SSI 発生率は 1.13％から 1.45％に上昇した。本研究で提案した方法は、正確であり、経時的に持続可能なようである。

監訳者コメント：
SSI の発生率の見積もりは、複数のデーターベースを併用した細かな解析によって精密になり、増えることは予想できる。この場合、データーベースの情報のどの部分を組み入れるかという基準、および組み入れた例が実際に妥当であるかという検証が、重要になってくるであろう。大きな地域よりも、限られた地域や、連携した医療機関のグループを扱うことの方が、このような取り組みにより適した規模かもしれない。
