Extended-spectrum β-lactamase Enterobacteriaceae (ESBLE) in intensive care units: strong correlation with the ESBLE colonization pressure in patients but not same species

基質特異性拡張型β- ラクタマーゼ産生腸内細菌科細菌（ESBL-E）による汚染のスクリーニングのため、集中治療室（ICU） 6 室のシンク排水管のサンプリングを行った。シンク排水管の汚染は高率（59.4％）であることが認められた。ICU ごとにデータを分析すると、⌈シンク排水管で分離された ESBL-E 種の数／サンプリングされたシンク排水管の総数⌋の比率は、⌈前年に ESBL-E の保菌が特定された患者の入院日数／前年の入院日数の総数⌋の比率と、高度に相関していた（Spearman 係数 0.87、P ＝ 0.02）。同時に、ESBL-E 種の分布は患者とシンク排水管で有意差があった。

監訳者コメント：
近年、多剤耐性グラム陰性桿菌の院内伝播においてシンクなどの水まわりの汚染が関係しているとする報告が多い。しかし本論文のように、患者から分離される菌と、水まわりから分離される菌が完全に同一というわけではないようである。プラスミドによる耐性遺伝子の伝播は従来ヒトの腸内で起きると考えられてきたが、環境も含めたプラスミドによる耐性遺伝子の伝播の可能性について考えた方が良いのかもしれない。
