Two-year analysis of Clostridium difficile ribotypes associated with increased severity

背景
クロストリジウム・ディフィシル( Clostridium difficile)リボタイプによっては、再発や重症化など複雑な疾患表現型と関連し、特に、よく記述されている強毒型 RT027 が関連している。本研究の目的は、感染症を引き起こすリボタイプのパターンと、感染した場合の重症度との関連を明らかにすることである。
方法
2011 年から 2013 年に C. difficile検査のために大規模な診断検査施設に提出されたすべての糞便サンプルにおいて、ルーチン検査および培養を実施した。すべての C. difficile株をリボタイプに分類し、関連する臨床・人口統計学的な患者データを回収し、リボタイピングデータと関連付けた。
結果
C. difficileの分離株 705 株から、計 86 種類のリボタイプが同定された。RT002 と RT015 の検出頻度がもっとも高かった(22.5%、159 株)。強毒型 RT027 は 5 株のみであった(0.7%)。臨床情報が入手できた患者 450 例のうち 90 例(20%)が、C. difficile分離後 30 日以内に死亡した。RT220 は、もっとも頻度が高い 10 種類のリボタイプの 1 つであり、本研究で検出された RT002 および RT015、これら以外の全リボタイプと比較して、C 反応性蛋白の中央値の増加、30 日全死因死亡率の有意な増加と関連した。
結論
多様な C. difficileリボタイプは、C. difficile感染症の症状に関与している。C. difficile関連死亡率は近年低下しているが、重症化と関連する系統株の蔓延は、今後の死亡率増加の前兆といえるかもしれない。より重症の疾患と関連する RT220 など新たな系統株のサーベイランス強化が、病原性を分析するゲノム学的アプローチとともに必要とされる。

監訳者コメント
遺伝子の系統により病原性の強い病原体を特定することができる場合、病原性の強い病原体が院内に入り込んできた場合の対策を強化し、感染のリスクを低減することができるようになるかもしれない。
