Top research priorities in healthcare-associated infection in the UK

背景
患者、介護者、医療従事者により重要とみなされる研究疑問と、多くの医療分野で実施される研究との不一致がみられる。患者や介護者の意見を取り込んで、医療関連感染（HCAI）における研究優先順位を評価した関連研究は、文献で特定されなかった。
目的
英国における HCAI の予防、診断、治療に関して研究の最優先順位を特定するために、Healthcare-Associated Infections Priority Setting Partnership を構築し、この全グループの意見を検討した。
方法
James Lind Alliance の優先順位づけの行使の原理におおむね従った。
結果
研究の優先順位について患者、介護者、医療従事者の意見を求め、次いで、この 3 グループの協議に対する 221 の有効回答から、計 259 の妥当性のある新たな研究疑問を特定した。優先順位づけパートナーシップの運営委員会は、50 の新たな疑問に対して優先順位を理論づけた。これらの疑問に対して、質の高い最近のシステマティックレビュー、すなわち、さらなる研究が必要であると結論づけられたものや、最近のシステマティックレビューの結論にもかかわらず、運営委員会がさらなる研究が必要であるとみなしたものはないことが、文献レビューにより立証された。中間調査で 50 の疑問を順位づけ、患者、介護者、医療従事者のグループディスカッションによる最終的な優先順位づけワークショップで、上位 32 の疑問から主要な 10 の研究の優先順位を合意により特定した。
結論
HCAI 関連の研究における上位 10 の優先領域を特定するために、James Lind Alliance の方法と原理による、患者、介護者、医療従事者を含めた優先順位決定プロセスを使用した。このような不確実性に対処するには、基礎研究、橋渡し研究、臨床研究、公衆衛生研究が必要とされるであろう。
