Molecular epidemiology of an outbreak of human parainfluenza virus 3 among oncology patients

血液腫瘍科の患者で発生したヒトパラインフルエンザウイルス 3 型の院内アウトブレイクを、4 週間にわたり 12 例について記録した。曝露歴およびヒトパラインフルエンザウイルス3 型の分子解析から、このアウトブレイク中には市中感染および院内伝播の両方が発生していたと推測した。パラインフルエンザウイルス3 型の分子解析からはさらに、腫瘍科病棟の複数の患者において、連鎖的に伝播が生じていたことが示唆された。そして伝播には媒介物の移動や訪問者、病棟内の活動が関連していたことが後に明らかとなった。この院内伝播は、感染対策チームによる高度な清掃手順を含めた介入の後、食い止めることができた。

ヒトパラインフルエンザウイルス（HPIV）は免疫不全者に感染した場合、重症な下気道感染症を生じうる。4 つの血清型があり、症状と疫学状況が異なるが、このうち 3 型（HPIV-3）は肺炎例や入院を要する例で最も割合が高い。今回の報告は、患者家族が親しくなり、お互いの親密な交流と物品の共有があったことが、一つの伝播要因であった。これまで、造血幹細胞移植患者での院内伝播や、呼吸器症状のない／軽微な患者からの伝播についても報告が相次いでいるが、本報告も参考になる事例と思われ、一読をお勧めする。
