Development of consensus-based international antimicrobial stewardshiP competencies for undergraduate nurse education

背景
看護師が抗菌薬適正使用支援に貢献することに関して、国内外の政策立案者による認識が高まりつつある。看護学生への教育は、看護師として抗菌薬適正使用支援の役割と実践に向けて準備させる絶好の機会となるが、抗菌薬適正使用支援の教育を組み入れているのは、看護教育プログラムの 3 分の 2 にとどまり、推奨される抗菌薬適正使用支援の原則をすべて網羅しているのは、わずか 12％であった。また、看護師は、抗菌薬に関する知識が十分ではないと報告しており、抗菌薬適正使用支援という用語を聞いたことがない者が多かった。
目的
看護学生に対する教育に適した抗菌薬適正使用支援コンピテンシーの説明指標に関する国際的なコンセンサスを規定すること。
方法
2 段階のオンライン調査からなる修正デルファイ法により、看護師の教育と実践における処方および薬剤管理の専門知識、ならびに抗菌薬適正使用支援の専門知識をもつ 15 名からなる国際的なパネルメンバーを招いた。2019 年 2 月から 3 月にデータを収集した。
結果
参加者 15 名が専門家パネルのメンバーになることに同意し、そのうち 13 名（86％）が 1 回目の質問票に回答し、この 13 名（100％）が 2 回目の質問票に回答した。コンピテンシーに関する包括的な 6 領域、ならびに抗菌薬適正使用支援の実践に必須の 63 項目の説明指標に関してパネルメンバーのコンセンサスが得られ、一貫して高水準の合意に達した。
結論
コンピテンシーの説明指標に関して高水準の合意に達したことから、看護学生に対する教育および有資格看護師（補助看護師など新たな役割を担う者も含む）の抗菌薬適正使用支援活動を方向付けるために、コンピテンシーの説明指標を使用すべきである。

APICでも本件は話題となっており、看護師が抗菌薬適正使用支援に関与できることが多く中心的な役割を担う一員たるべきであると考えられている。診療報酬加算の要件にも看護師が組み入れられており活躍が期待される一方で看護師の抗菌薬に関する教育が課題である。
