Effects of prophylactic use of taurolidine-citrate lock on the number of catheter-related infections in children under 2 years of age undergoing surgery

中心静脈アクセスにはカテーテル関連感染症（CRI）の発症リスクがある。本研究の目的は、タウロリジン・クエン酸溶液の予防的な使用が CRI の発生数に及ぼす影響を評価することであった。小児 86 例に 97 本のカテーテルが挿入されたが、これを無作為に以下の 2 群に分けた：標準的な無菌操作を受けた T（－）群（N ＝ 49）と、それに加えて経静脈栄養や薬剤投与の合間にタウロリジン・クエン酸溶液でラインを満たした T（＋）群（N ＝ 48）である。CRI の発生は、T（－）群は 16 件、T（＋）群は 1 件であり、この差は有意であった（P ＜ 0.05）。タウロリジン・クエン酸溶液の使用は、CRI 予防において安全かつ効果的な方法のようである。

監訳者コメント：
タウロリジン・クエン酸はアミノ酸のタウリンに由来し、これまでロック療法として用いた場合、細菌・真菌のカテーテル感染が減少する一方、本剤の投与による一過性の副作用は 8％未満とも言われていた。本研究では 2 歳未満の小児に、1 日 2 時間以上ロック療法を行った際の有効性を判定している。一方で Journal of Hospital Infection (2012) 80, 304-309.では血液悪性腫瘍の小児を対象とした成績が報告されており、こちらも合わせて参照されたい。バイオフィルム抑制作用が効果を高めた可能性があり、この点がより明らかにされることに期待したい。
