本稿では、クロアチアの河川水における肺炎桿菌カルバペネマーゼ(KPC)‐2 産生肺炎桿菌(Klebsiella pneumoniae )の最初の症例について報告する。全体で、KPC‐2 産生肺炎桿菌の分離株 4 株について分析した。すべての分離株が同じ遺伝的系統を持ち、ST258 に属していた。分離株は、コリスチンに感受性を示す同一の多剤耐性プロファイルを示した。全分離株で、blaSHV-1aac(3’)-IIaac(6’)-Ibaph(3’)-Ia 遺伝子が検出された。また、サイズの異なる(約 140、230、225、220 kb)非接合性の単一 IncFII プラスミド上に、blaKPC-2 遺伝子が局在した。肺炎桿菌は河川水に最長 50 日間は生存可能であり、環境内に存続し拡散する能力が確認された。

監訳者コメント
ここ数年、河川、海などの環境における耐性菌の状況が各地で調査され、その存在が確認されている。One Health という視点で考え、多方面の専門家がともに対策を検討すべき課題であると思われる。
