Screening for carbapenemase-producing Enterobacteriaceae in previous carriers readmitted to hospital: evaluation of a change in screening policy

英国の病院において、カルバペネマーゼ産生腸内細菌科細菌（CPE）は、ますます深刻な問題となっている。伝播の予防には早期発見が必要である。本研究では、発生率がより高い病院 1 施設において、blaKPC 関連 CPE の保菌歴のある患者のスクリーニングの新たな方針について評価する。従来の方針は、「一度陽性になると、陽性が持続」を前提としていた。新たな方針では、迅速なスクリーニングとリスク評価を使用する。blaKPC 関連 CPE 保菌歴のある大部分の患者（76.5％）で、再入院時またはその後の入院中に blaKPC 関連 CPE が検出できなかった結果が示されたが、初回の結果が陰性であっても、継続的なスクリーニングが必要であることが示された。新たな方針では、リスクに基づくアプローチを適用し、発生率のより高いトラストにおいて隔離設備を優先させる。

監訳者コメント：
CPE 保菌歴のある患者の再入院にあたり、データを解析し、これまでに「一度陽性になると、陽性が持続」すると考えた対策から、繰り返しスクリーニングを行い、検出された場合に対策を実施する、にポリシーを変更した、という論文である。これに伴い、隔離のための病室の不足、不要な対策にかかる費用が軽減されると思われるが、長期データの解析による評価が必要であろう。
