Isopropanol at 60% and at 70% are effective against ‘isopropanol-tolerant’ Enterococcus faecium

エンテロコッカス・フェシウム（Enterococcus faecium ）ATCC 6057、ST 796（イソプロパノール耐性株）、ならびにエンテロコッカス・ヒラエ（Enterococcus hirae ）ATCC 10541について、イソプロパノールの殺菌能を調べた（EN13727）。60％および 70％イソプロパノールは、15 秒ですべての菌株に対して有効（≧ 5.38 log₁₀ 減少）であったが、23％イソプロパノールは無効であった（≦ 15 分で＜ 0.99 log₁₀ 減少）。続いて4 フィールドテストを用い、E. faecium に対する 70％イソプロパノールの効果を試験した。8 mL は 1 分では十分な効果がみられなかったが（＜ 5 log₁₀ 減少）、16 mL は有効であった（≧ 5.85 log₁₀ 減少）。使用量が適切であれば、60％および 70％イソプロパノールは E. faecium に対して有効であると安心してよいであろう。

監訳者コメント：
昨年、23％イソプロパノールに耐性を示す E. faecium がオーストラリアから報告され、さらにスイスではこの中の ST796 が侵襲性感染症の集団発生を生じていたことが明らかになった。加えてこの E. faecium は 70％イソプロパノールによる表面消毒にも抵抗性を示した。本報告は、これらの報告を追試する意味もあり、十分な濃度と接触時間を確保すれば60％～ 70％のイソプロパノールはいまだ有効であるとの結果が得られた。ただしイソプロパノール・アルコール耐性の機序はまだ不明といえ、今後も詳細な検討とデータの集積が必要である。
