世界保健機関は 2016 年に手術部位感染症を予防するためのガイドラインを発表した。本ガイドラインは、層流換気システムは関節全置換術後の感染症リスクを減らすために使用する必要はないという、条件付きの推奨を含んでいた。この推奨は主に、病院感染サーベイランスレジストリの情報のシステマティックレビューおよびメタアナリシスに基づいていた。この推奨は、Charnley と英国医学研究協議会によって先に実施された主要な研究において発表された情報と相反している。本論文の第 1 の目的は、医学研究協議会の研究を再考および説明し、その研究に対する批判に答えることである。第 2 の目的は、最近の一部の研究で、超清浄空気システムが関節全置換術後の関節深部感染を減らすことを実証できなかった理由を示すことである。本論文では超清浄空気システムによって微生物保有粒子の平均大気中濃度を 10/m3 未満に、また望ましくは 1/m3 未満にすれば、関節全置換術後の関節深部感染は、従来の方法で換気された手術室よりも少なくなることを明らかにしている。

本論文は、人工関節置換術において、層流換気システムが SSI 減少を目的として使用されるべきではないという 2016 年の WHO ガイドラインの推奨に対する反論（つまり、層流換気システムは SSI 予防に有用であると主張したい）を目的としている。この議論に興味のある読者は一読してみてはどうだろうか。
