Emergence of Candida auris in Russia

本稿では、モスクワの病院 1 施設の集中治療室におけるカンジダ・オーリス（Candida auris）感染症の出現について報告する。2016 年から 2017 年にかけて49 症例が診断を受け、リスク因子と抗真菌感受性について記述が行われた。適切な抗真菌薬療法を受けなかった患者の血流感染症 19 件において、30 日全死因死亡率は 42.1％であった。内部転写スペーサーおよび D1/D2 領域,ならびに ERG11 遺伝子における K143R 置換を対象とした系統樹解析から、調査対象とされた C. auris 株は南アジア起源であることが示された。このロシアにおける C. auris 感染症の初の一連の症例報告は、このカンジダ種が急速に拡散していること、また国際的なサーベイランスおよび制御策が必要であることを示している。

監訳者コメント：
カンジダ・オーリスは、2005 年日本で発見された真菌性新興病原体である。院内感染と強く関連しており、2016 年 6 月 24 日米国 CDC は本菌が患者から検出されないか注意深く監視するように警告を発し、全世界が注目した。本菌はフルコナゾール耐性のみならず多剤耐性であり、カンジダ血症をおこすため死亡率も高く、アウトブレイクすると感染対策上非常に厄介な真菌である。また、通常の生化学的性状による同定は不可能で、遺伝子もしくは質量分析による同定が必要である。現時点では日本での本菌によるアウトブレイクは報告されていないが、引き続き警戒する必要がある。
