血液培養からの抗菌薬耐性遺伝子の検出:性能評価および抗菌薬療法に対する影響★

遺伝子検査は、タイムリーな薬剤耐性の予測や、エンピリックに開始した抗菌薬の最適化を行うにあたり、妥当な一法となる可能性がある。本研究では、主要なカルバペネマーゼと基質特異性拡張型β – ラクタマーゼ(ESBL)の遺伝子、および黄色ブドウ球菌(Staphylococcus aureus)特異的遺伝子と mec 遺伝子を血液培養から 3 時間未満で検出できる ELITe MGB アッセイの性能を評価した。遺伝子検査の結果と、従来の表現型に基づいた結果には、良好な一致が認められた。
診療記録の後向き分析により、ESBL 産生菌、カルバペネマーゼ産生腸内細菌科細菌、MRSAによる血流感染のおよそ 50%は、当初は無効な薬剤で治療されたことが明らかになった。全体として、もし遺伝子検査が用いられていれば、36.3%の患者は少なくとも 24 時間早く適切な治療を受けることができたであろう。

ELITe MGB(ELITe InGenius)は、核酸抽出からマルチプレックスreal-timePCR、判定まで3 時間以内に行うことができるキットであり、海外では各種微生物および耐性遺伝子に特異的なセットが発売されている。近年、簡便に複数の微生物・耐性遺伝子を検出することができる遺伝子検査の導入の速度が速くなり、我々が持つ選択肢も確実に増加している。検査法それぞれの評価もさることながら、どのような場面で、どのように使用し、治療と感染対策の両面について、結果を現場でのマネジメントにいかにつなげていくかが、重要である。使用される状況は医療機関によっても異なるが、このような目的・方針のもとで、本キットが有力な手段となることに期待したい。
