The economic impact of antimicrobial stewardship programmes in hospitals: a systematic literature review

背景
抗菌薬適正使用支援プログラム（ASP）には、臨床現場での合理的な感染症治療における持続可能な管理を可能にする対策も含まれる。ASP 導入の成功には、病院管理を含む、さまざまな医療従事者の各領域間での綿密な連携が必要である。これまで、ASP は、臨床‐薬理学的見地および感染症の観点から主に評価されてきた。
目的
ASP の経済的影響において決定的に重要なパラメーターを特定し、評価すること。
方法
ASP と関連する医療経済研究のピアレビュー論文の系統的検索を実施した。主要評価項目は、薬剤費の節減および病院の収益減少の軽減とした。
結果および結論
計 16 件の研究がすべての選択基準に合致した。公表済みの臨床試験によるエビデンスの大部分で、抗菌薬の直接経費減少により節減が確認された。ASP がもたらす収益効果を、入院期間および再入院率の減少によって立証する研究もある。

監訳者コメント：
ASP により適正な抗菌薬療法が行われた結果、処方総量が増加することはありえる。一方で合併症や患者予後については、集中治療を受けている場合感染症の影響なのかそれ以外の疾患の関係で予後が変化しているのか識別しづらいこともある。
