Laboratory exposure to Coccidioides: lessons learnt in a non-endemic country

コクシジオイデス（Coccidioides）は主要な病原性真菌の 1 つであり、感染性の高い分節型分生子を介してヒトに感染し、生命を脅かす播種性感染症を含む相当な罹患率の原因となっている。感染用量が低いため、検査技師は作業中に感染する可能性がある。その結果、コクシジオイデス症は世界的に最も頻度が高い検査室感染の全身真菌症として報告されている。コクシジオイデス症の診断が疑われることの少ない非流行国においては、このリスクは増大する。今回我々は、チリの微生物検査室において、44 名がコクシジオイデス・ポサダシ（Coccidioides posadasii）に不用意に曝露を受けたこと、そして MALDI-TOFを用いて迅速に同定し、封じ込めの手段を講じたこと、ならびに非流行地域という状況で学んだ教訓について報告する。

監訳者コメント：
コクシジオイデスはバイオセーフティーレベル（BSL）3 に相当する。非流行国である本邦においても、微生物検査室で未同定のコクシジオイデスを培養してしまい、曝露を受けてしまうリスクは、本報告と同様に存在する。一方で、臨床医が渡航歴や病歴から、コクシジオイデス症を早期に疑って、微生物検査室に相談の連絡が入る例もある。監訳者も、「米国南西部から帰国後の胸部異常影の精査で、気管支肺胞洗浄液の採取前に微生物検査室に連絡が入り、最終的に本症と判明したものの、検査室での曝露を未然に防ぐことができた」例と、「難治性耳感染で来院したために、通常の扱いで培養検体が提出されてしまった」例を経験している。診療科にかかわらず、渡航と本症に関わる認識を高める工夫も必要であるといえる。
